29 CFR  1910.1030(c)(1):1030(c)(1)(i): At this establishment, the employer did not establish a written blood borne pathogen exposure program designed to eliminate or minimize employee exposure when treating injuries.  On or about September 4, 2020, employees and management officials received potential occupational exposure to blood and other potentially infectious materials while cleaning droplets and pool(s) of blood following a work related accident involving a hand injury on an Omar Hinge Boring-Inserting Machine/Drill Panel in the Production Shop. The employer did not establish a written bloodborne pathogen exposure control plan to eliminate or minimize employees' exposures to bloodborne hazards.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.1030(f)(1)(i): The employer did not make available the hepatitis B vaccine and vaccination series to all employees who have occupational exposure, and post-exposure evaluation and follow-up to all employees who have had an exposure incident.  On or about September 4, 2020, employees and management officials received potential occupational exposure to blood and other potentially infectious materials while cleaning droplets and pool(s) of blood following a work related accident involving a hand injury on an Omar Hinge Boring-Inserting Machine/Drill Panel in the Production Area. The employer did not ensure that the Hepatitis B vaccine and vaccination series were made available and offered to the employees with occupational exposures to bloodborne hazards.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.1030(g)(2)(i): The employer did not train each employee with occupational exposure in accordance with the requirements of this section. Such training must be provided at no cost to the employee and during working hours. The employer shall institute a training program and ensure employee participation in the program.  On or about September 4, 2020, employees and management officials received potential occupational exposure to blood and other potentially infectious materials while cleaning droplets and pool(s) of blood following a work related accident involving a hand injury on an Omar Hinge Boring-Inserting Machine/Drill Panel in the Production Area. The employer did not ensure employees were trained during his/her initial assignment where an occupation exposure may take place.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.212(a)(3)(ii): Point(s) of operation guards were not designed and constructed to prevent the operator from having any part of his/ her body in the danger zone during the operating cycle:  On or about September 4, 2020, employees were exposed to amputation hazards and hand injuries while operating an Omal Hinges Boring-Inserting Machine/Panel Drill in the Production Shop and the employer did not install an adequate guard to eliminate amputation hazards near the point of operation.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d)  The Chicago Storage Solutions, Inc dba Closets By Design was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1158387, citation number 1, item number 1 and was affirmed as a final order on August 3, 2016, with respect to a workplace located at 850 Ridgeway Unit A, Aurora, IL. 60506.

29 CFR 1904.29(a): A Log of all Work-Related Injuries and Illnesses (OSHA Form 300), and/or Summary of Work-Related Injuries and Illnesses, (OSHA Form 300-A), and/or the Injury and Illness Incident Report (OSHA Form 301) or equivalent forms were not kept by the establishment:   On or about September 4, 2020, an employee received broken bones and a nicked artery on the left hand while operating an Omal Hinges Boring-Inserting Machine/Panel Drill in the Production Shop, which resulted in days away from work and the employer did not maintain an adequate  OSHA 300 Log for the calendar year of 2020 of any and all recordable work-related injuries and illnesses.   There is no abatement certification or documentation required for this item.  The Chicago Storage Solutions, Inc dba Closets By Design was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1904.29(a), which was contained in OSHA inspection number 1158387, citation number 2, item number 1 and was affirmed as a final order on August 3, 2016, with respect to a workplace located at 850 Ridgeway Unit A, Aurora, IL. 60506.
