1910.109(e)(1)(iv): Persons authorized to prepare explosive charges or conduct blasting operations did not use every reasonable precaution, including but not limited to warning signals, flags, barricades, or woven wire mats to insure the safety of the general public and workmen. a) Economizer 2 Top - Employees performing blast cleaning operations were exposed to struck by hazards from not being protected by an adequate barricade or woven wire mat, on or about September 29, 2020. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

Before a blast is fired, the employer did not require that a loud warning signal be given by the person in charge, who did not make certain that all surplus explosives were in a safe place, all persons and vehicles were at a safe distance or under sufficient cover, and that an adequate warning was given. a) Economizer 2 Top - Prior to the warning signal the person in charge did not make certain that all persons were at safe distance or under sufficient cover, on or about September 29, 2020.
