29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about September 26, 2020, during the installation of gutters and downspouts on a job site located at 7189 Cody Ct, Mentor OH 44060, the employer did not ensure employees working on a roof of a single family home were protected from a fall of approximately 10 feet by utilizing a fall protection system.   All Construction Services, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b) (13) Duty to have fall protection), which was contained in OSHA inspection number 1276245, citation number 001, item number 001 and was affirmed as a final order on 12/07/2017, with respect to a workplace located: 10110 Corbetts Ln, Twinsburg, OH 44087  All Construction Services, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b) (13) Duty to have fall protection), which was contained in OSHA inspection number 1197225, citation number 01, item number 001B and was affirmed as a final order on 01/12/2017, with respect to a workplace located: 19032 Ledgestone Dr, Strongsville, OH 44149  All Construction Services, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b) (13) Duty to have fall protection), which was contained in OSHA inspection number 1103523, citation number 01, item number 002 and was affirmed as a final order on 12/23/2015, with respect to a workplace located: 2550 University Dr SE, Massillon, OH 44646

29 CFR  1926.1053(b)(4):Ladder(s) were used for purposes other than the purpose for which they were designed:   On or about September 26, 2020, at the job site located at 7189 Cody Ct, Mentor OH 44060; during the installation of gutters and downspouts on a residential roof, the employer did not ensure that employees utilized stepladders (self-supporting ladder) as they were designed for use. Employees utilized stepladders as a non-self-supporting ladder on a roof to gain access to an elevated working surface.    All Construction Services, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.1053(b)(4) Ladders shall be used only for the purpose for which they were designed), which was contained in OSHA inspection number 1197225, citation number 01, item number 003 and was affirmed as a final order on 01/12/2017, with respect to a workplace located: 19032 Ledgestone Dr, Strongsville, OH 44149

On or about September 26, 2020, during the installation of gutters and downspouts on a jobsite located at 7189 Cody Ct. Mentor, OH 44606, the employer did not ensure employees using a ladder did not carry an object or load that could cause the employee to lose balance and fall.
