On or about October 8, 2020, at the above addressed jobsite, an employee required to operate a GRADALL 534D-42 Telehandler had not been reevaluated on the operator's performance within the last three years, thereby exposing employees to struck by and crushing hazards.

On or about October 8, 2020, at the above addressed jobsite, an employee performing carpentry operations was standing on the top step of an eight (8) foot A-Frame ladder, thereby exposing the employee to hazards associated with falls.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a.  On or about October 8, 2020, at the above addressed jobsite, employees performing residential construction operations, nineteen (19) feet  above the ground, were not protected from falls, thereby exposing the employees to fall hazards.  b. On or about October 8, 2020, at the above addressed jobsite, an employee performing residential construction operations, fourteen (14) feet three (3) inches above the ground, was not protected from falls, thereby exposing the employee to fall hazards.  c.  On or about October 8, 2020, at the above addressed jobsite, employees performing residential construction operations, twelve (12) feet two (2) inches above the ground, were not protected from falls, thereby exposing the employees to fall hazards.  Forestview Construction Services LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1298244, citation number 1, item number 1a, and was affirmed as a final order on April 11, 2018, with respect to a workplace located at Lot 4 Daniel Lewis Trail in New Lenox, IL.

29 CFR  1926.503(b)(1):The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a.  On or about October 8, 2020, at the above addressed jobsite, employees had been provided fall protection training, however the employer failed to keep a written certification of record.
