The path to ground from circuits, equipment, and enclosures were not  permanent, continuous, and effective.  a) Maintenance shop on captive barge - On or about October 8, 2020, workers were using a Miller Millermatic 140 Auto-Set Welder, SN MD100290N, that was not grounded exposing them to a hazard of electrical shock.

29 CFR  1910.305(a)(2)(xi): Cable assemblies and flexible cords and cables were not supported in place at intervals that ensure that they will be protected from physical damage.    a)  Gangway adjacent to property at 2415 Pier Street.  On or about October 8, 2020, the cord to the vending machine was not supported at the plug resulting in damage from pressure created by the weight of the cord and exposure to the environmental elements exposing workers to an electrical shock hazard.
