29 CFR  1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):   (a) Warehouse:  On or about 9/15/2020, and at times prior, the employer exposed employees to amputation hazards when operating the Amada Hydraulic Press Brake(HFE 170 S/N 060247) in that the point of operation was not guarded while bending sheet metal used to fabricate racks or panels.

29 CFR  1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  Facility: The employer did not notify OSHA until 9/30/2020 of an accident that occurred on 9/15/2020, which caused the amputation to the first index finger on the right hand.
