29 CFR 1910.334(a)(2)(i):Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): On or about 11/09/2020, at the northwest bay area inside the warehouse, employees were exposed to electrical hazards such as , but not limited to burns, as a result of not inspecting damaged cords that include a 25', 14 gauge cord and a 25 ' 12 gauge cord. The employees were not inspected prior to use.

29 CFR 1910.334(a)(2)(ii):There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service: 1. On or about 11/13/2020, in the warehouse area on the H2O Truck, 25' , 12 gauge cord with damage to the outer sheathing, exposing employees to electrical hazards to include, but not limited to, burn injuries. The damaged cord had not been removed from service. 2. On or about 11/13/2020, in the warehouse at the north west bay area a 25' , 14 gauge cord that had a ground prong missing, exposing employees to electrical hazards to include, but not limited to, burn injuries. The damaged cord had not been removed from service.
