29 CFR  1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  (a) 3E Concrete, LLC at 3519 E 1/2 Rd., Palisade, CO: On and around November 17, 2020, the exposing employer did not instruct each employee in the safe operation of a Stihl TS420 saw. An employee, who was distracted, failed to tighten the cap of the saw, while fueling it. The employee started cutting rebar, when the cap became dislodged and flammable liquid spilled on his right leg. This condition resulted in the employee suffering burns on his right leg and hip, when the flammable liquid was ignited by sparks from cutting rebar.
