29 CFR  1910.136(a):The employer did not ensure that each affected employee uses protective footwear when working in areas where there is a danger of foot injuries due to falling or rolling objects, or objects piercing the sole, or when the use of protective footwear will protect the affected employee from an electrical hazard, such as a static-discharge or electric-shock hazard, that remains after the employer takes other necessary protective measures:  a.  At the manufacturer's site located at  1811 SW 42nd Ave. Ocala,  FL, employees on or about December 3, 2020, were exposed to crushing and laceration hazards when operating a 500 Lbs. metal bin cart with plastic debris without any foot protection provided.

29 CFR  1910.147(c)(6)(i)(C):Where lockout was used for energy control, the periodic inspection did not include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being insp.  a.  At the manufacturer's site located at  1811 SW 42nd Ave. Ocala,  FL, employees on or about December 3, 2020, were exposed to energy release hazards when employer did not inspect all of the extrusion line and waste handling machine's lock and tag out procedures with all employees using the process.
