29 CFR  1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section:  (a) On or about December 2, 2020, at the construction site located near W186N9581 Bancroft Drive, Menomonee Falls WI, an employee was uncovering the water main in an approximately 7.7 feet deep excavation without adequate cave-in protection, thereby exposing the employee to a struck-by /crushing hazard.  Dorner, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1348352, citation number 1, item number 2a and was affirmed as a final order on November 21, 2018, with respect to a workplace located at Plymouth and Roosevelt St Oshkosh, WI 54901.  Dorner, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1426704, citation number 1, item number 1 and was affirmed as a final order on November 19, 2019, with respect to a workplace located at 1823 Plymouth St Oshkosh, WI 54901.  Dorner, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1415393, citation number 2, item number 1 and was affirmed as a final order on January 24, 2020, with respect to a workplace located at Bridge St / 17th St, Grafton, WI 53024.
