Instances B:  On or about December 10, 2020, an employee operated a Ingersoll-Rand Rough Terrain Forklift, Model Number VR-843, Serial Number 163628, without completing the training requirements by 29 CFR 1910.178(l).

29 CFR  1926.251(e)(4)(ii): Nylon slings were not kept free of all sharp edges that could in any way damage the webbing:   On or about December 10, 2020, during a connecting operations with a 1,300 pound structural steel gird, two nylon slings were not protected from the sharp edges of the forks of a Genie Telehandler Forklift.

29 CFR  1926.753(d)(2)(i): Materials being hoisted were not rigged to prevent unintentional displacement.  On or about December 1, 2020, the employer hoisted structural steel using slings suspended from the forks of a Genie Telehandler Forklift (Model GTH844D, Serial Number  0814-18258), the load was not rigged to prevent unintentional displacement from the forks.
