29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles. On or about December 10 2020, one employee was performing punch out tasks to the side of a building using a pneumatic air gun in an aerial lift. The employee was approximately 15 feet high and not wearing eye protection, exposing the employee to the hazard of being struck-by flying objects.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about December 10, 2020, one employee was performing framing activities inside an aerial lift without wearing head protection. The aerial lift was positioned within 6 feet of the building where roofing work was being performed, approximately 15 feet above the employee, exposing the employee to the hazard of being struck by falling objects.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: On or about December 10, 2020, two employees were performing punch out tasks to the side of a building using a pneumatic air gun in an aerial lift. The two employees were approximately 15 feet high and not wearing fall protection, exposing the employees to a fall hazard.
