Portable cord and plug connected equipment and flexible cord sets (extension cords) shall be visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). Cord and plug connected equipment and flexible cord sets (extension cords) which remain connected once they are put in place and are not exposed to damage need not be visually inspected until they are relocated.  29 CFR  1910.334(a)(2)(ii): If there is a defect or evidence of damage that might expose an employee to injury, the defective or damaged item shall be removed from service, and no employee may use it until repairs and tests necessary to render the equipment safe have been made.  At the facility located at 525 South Green Bay Road, Neenah, Wisconsin, inside of the Detailing Area, the employer did not ensure that flexible cords were inspected prior to the start of each shift. The cord jacket of a flexible cord in use at the South Detail Station was observed to be damaged, exposing employees to electrical hazards.

At the facility located at 525 South Green Bay Road, in Neenah, Wisconsin, inside of the Detailing Area, employees in the Detailing Area were exposed to chemical hazards while using cleaning chemicals in spray bottles, without the required form of label or markings. Employees were exposed to chemicals hazards in the following instances:  a) Five spray bottles containing chemicals did not have the required labeling at the North Detail Station  b) One spray bottle containing chemicals did not have the required labeling at the South Detail station
