29 CFR 1926.300(b)(1):Power operated tool(s), designed to accommodate guards, were not equipped with such guards when in use: (a) Jobsite: On or about December 30, 2020, an employee was using a DeWalt hand held grinder and the grinder was not fitted with its guard. The employee was grinding out concrete from between bricks to a residential home to repoint it.

29 CFR 1926.451(g)(2):The employer did not provide fall protection for employees erecting or dismantling supported scaffolds: (a) Jobsite: On or about December 30, 2020, the employer did not provide fall protection for an employee who was dismantling a tubular welded frame scaffold. The fall hazard was about 12 feet to the ground.

29 CFR 1926.451(c)(2):Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation: (a) Jobsite: On or about December 30, 2020, an employee was working from a tubular welded frame scaffold and two uprights did not bear on base plates nor mud sills or other adequate firm foundation. The two uprights had sunk into the earth by about 4-6 inches.
