Specific procedural steps were not included for shutting down, isolating, placement of lockout devices and specific requirements for testing to verify lockout/tagout devices and other energy control devices on the uncoiler, slitter, and recoiler.   b. On or about December 21, 2020, located Strapping Line #2, the employer did not clearly and specifically outline the rules and techniques to be utilized for the control of hazardous energy and the means to enforce compliance for all the interconnected equipment of the Strapping Line, thereby exposing the employees to the hazards associated with performing service and maintenance activities on the Strapping Line equipment. On January 5, 2021, an employee placed a folded piece of paper into the ingoing nip point on a recoiler's coil material as it rotated. The employee was pulled into the recoiler resulting in death.
