29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles: On or about 01/06/2021 during the installation of roofing material at the job site located at 123 Cumming Street, Alpharetta, GA, the employer failed to ensure that employees installing roof trusses to a newly constructed home were provide with and/or utilized protective eyewear. Employees using pneumatic nail guns to secure roofing material to the structure without using safety eyewear were potentially exposed to struck by hazards and/or hazards created by flying debris.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: (a) New residential construction located at 123 Cumming Street, Alpharetta, GA 30009, First floor to second floor: On or about 01/06/2021, the employer exposed employees to fall hazards of approximately 10-feet, 7-inches between levels, in that safe access was not provided to employees engaged in framing activities on multiple levels of the structure. Employees used 2x4 cleats nailed to the framed walls to access each level. (b) New residential construction located at 123 Cumming Street, Alpharetta, GA 30009, Second floor to third floor: On or about 01/06/2021, the employer exposed employees to fall hazards of approximately 10-feet, 7-inches between levels, in that safe access was not provided to employees engaged in framing activities on multiple levels of the structure. Employees used 2x4 cleats nailed to the framed walls to access each level.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: 29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles: On or about 01/06/2021, during the installation of roofing material (trusses) on a newly constructed residential home located at 123 Cumming Street, Alpharetta, GA, the employer failed to ensure that employees were protected from a fall hazard by conventional fall protection and/or devices. Employees were exposed to fall hazards from heights of approximately 31-feet, 5-inches, from the top plate and on the top of the trusses to ground level. AMB Framing Services LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1414708, citation number 1, item number 1 and was affirmed as a final order on 07/24/2019, with respect to a workplace located at 6291 Jasper Way, Atlanta, GA 30331.
