29 CFR  1910.28(b)(1)(i):Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  a) Sorting Machine; On or about January 4, 2021 and at times prior to the employer exposed employees to fall hazards in that employees would climb out onto the sorting machine to conduct activities such as straightening boards and replacing chains while being approximately 19' above the ground level without the use of fall protection being enforced exposing the employees to fall hazards.

29 CFR  1910.124(g)(2):Employees who  worked with liquids that could burn, irritate, or otherwise harm their skin were not provided with an emergency shower and eye-wash station close to the dipping and coating operation:  a) Dip Tank; On or about January 4, 2021 and at times prior to the employer exposed employees to hazards associated with corrosive chemicals in that the employer required employees to work with and around a dip tank that contained corrosive chemicals to treat pallets of wood without first providing an emergency shower and eye-wash station exposing the employees to hazards associated with corrosive chemicals.

29 CFR  1910.265(c)(18)(i): Pulley(s), sprocket(s), sheave(s), drum(s) and block(s) of conveyor(s) were not arranged to prevent the possibility of injury due to hands or parts of clothing being caught between the belt(s) and pulley(s) or chain(s) and sprocket(s):  a) Stacker Conveyer; On or about January 4, 2021 the employer exposed employees to caught in hazards in that they allowed employees to work at or near shafts that operated the conveyer of the stacker system with unused keyways that were not guarded exposing the employees to caught in hazards.

29 CFR  1910.303(g)(2)(i):Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):  a) Twin Saw MCC Room; On or about January 4, 2021 and at times prior to the employer exposed employees to electrical hazards in that the employer failed to ensure that cabinets containing live electrical parts operating at 110 volts remained closed exposing employees to electrical hazards.
