29 CFR 1910.144(a)(1)(iii): Stop. Emergency stop bars on hazardous machines such as rubber mills, wire blocks, flat work ironers, etc., shall be red. Stop buttons or electrical switches which letters or other markings appear, used for emergency stopping of machinery shall be red. On or about December 28, 2020 and times prior thereto, the employer failed to ensure the emergency stop buttons on the Rotar Hopper Slotter machine were readily identified as such and was red in color.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. On or about December 28, 2020 and times prior thereto, the employer failed to guard the ink trough where employees add ink to the Rotary Slotter Machine exposing employees to amputation hazards.

Such tools shall not be in lieu of other guarding required by this section, but can only be used to supplement protection provided. On or about December 28, 2020 and times prior thereto, the employer failed to provide a larger tool to assist employees in adding ink to the Rotary Slotter Machine without placing their hand in the danger zone.

On or about December 28, 2020 and times prior thereto, the employer failed to ensure safety data sheets for the ink are provided in the native language of the operator; so that they know the hazards associated with that chemical.
