29 CFR  1926.451(c)(2)(v):The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied.   a) On or about January 19, 2021, at the previously addressed job site, a JLG Skytrak Telehandler was used to support a work platform while an employee was installing exterior windows and the platform was not secured to the JLG SKytrak Telehandler.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1926.454(a):The employer did not have each employee who performs work while on a scaffold(s) trained by a person qualified in the subject matter to recognize those hazards associated with the type of scaffold(s) being used and to understand the procedures to control or minimize those hazards.  a) On or about January 19, 2021, at the previously addressed job site, a JLG Skytrak Telehandler was used to raise a work platform.  Employees worked from the platform to perform construction activities such as installing windows and Tyvek house wrap.  Training from a person qualified in the subject matter of scaffolding was not provided to the employees working from the platform.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
