29 CFR 1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) At 507 Amos Street, Crestview, Florida: On or about January 12, 2021 and at times prior; the employer allowed his employee to transit a second floor walkway without guardrails installed or without any other type of fall protection exposing his employee to a fall hazard of 9 feet 9 inches to the ground below.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a) At 507 Amos Street, Crestview, Florida: On or about January 12, 2021 and at times prior; the employer exposed his employee to a fall hazard of 9 feet 9 inches to the ground below in that the employer did not ensure floor holes on a second floor walkway were guarded when employees were required to utilize the walkway.

The employer reported the hospitalization on January 19, 2021.
