29 CFR  1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest system:   a) Fourth Floor Roof: On or about February 18, 2021 and at times prior thereto employees were exposed to 12 to 24 foot fall hazards while working from a flat roof. 29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  a) Fourth Floor Roof: On or about February 18, 2021 and at times prior thereto employees were exposed to 15 to 40 foot fall hazards while installing tongue and groove roofing material on a 2/12 pitch roof, and were not connected to their vertical lifeline, or self-retracting lanyard.  NOTE-Abatement documentation and certification are required for this item.
