Employees were required to wear 3M model 07182 half mask air purifying respirators with P100 filters when welding, spray painting, or grinding. Employees performed maintenance of buses.  The buses were raised using auto lifts during maintenance.  Employees were exposed to mechanical, electrical, and potential energy sources of the raised buses.  The LOTO procedure was last inspected 1/15/2020.  This condition exposed employees to uncontrolled energy hazards.  (b)  Transdev Services, Inc., at 7500 E 41st Avenue, Denver, CO:  On and preceding 2/17/2021, the employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed.  Employees and contractors performed maintenance of a Champion Advantage air compressor.  The air compressor had electrical and pneumatic energy sources. The compressed air was not bled from the air compressor prior to performing service.  The employer did not ensure that employees recognized the compressed air as a hazardous energy source and did not train employees to relieve the stored energy prior to servicing the air compressor.  This condition exposed employees to an uncontrolled energy hazard.

Employees used Zep DZ-7 Neutral Disinfectant Cleaner to disinfect surfaces.  The disinfectant was placed in spray bottles for use.  The secondary containers were not provided with labels nor were they marked with the identification or hazard information of the contents.  This condition exposed employees to a chemical hazard.  (b)  Transdev Services, Inc., at 7500 E 41st Avenue, Denver, CO:  On and preceding 2/17/2021, the employer did not  ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier, signal word, hazard statement, pictogram, and precautionary statement.  An employee fueled buses from an approximate 10000 gallon tank.  The tank was labeled "Flammable" and "Unleaded".   The content of the tank was not identified nor was complete hazard information provided.  This condition exposed an employee to a chemical hazard.

Employees used chemicals, including but not limited to Simple Green All Purpose Cleaner, Zep Glass Cleaner, Zep DZ-7 Neutral Disinfectant Cleaner, and Round Up herbicide, at the office building of the complex.  SDSs for chemicals stored or used at the office building were not accessible in the office building.  This condition exposed employees to a chemical hazard.
