1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about February 25, 2021, on the top of a bentonite mixer located at Treviicos South worksite in Clewiston, Florida, the employer did not provide and install a fall protection system where an employee was exposed to a fall hazard of approximately 8 feet, 1 inches while repairing a leak on a keystone valve powered by a pneumatic rotary actuator.

1926.417(a): Controls that were to be deactivated during the course of work on energized or de-energized equipment or circuits were not tagged:  On or about February 25, 2021, at the surrounding area of the bentonite mixer located at Treviicos South worksite in Clewiston, Florida, the employer did not establish a LOTO program where employees were exposed to amputation hazards due to not de-energizing the pneumatic rotary actuator  while performing routine maintenance.
