29 CFR 1910.22(d)(2): The employer did not ensure that hazardous conditions on walking-working surfaces are corrected or repaired before an employee uses the walking-working surface again. The employer did not ensure that if the correction or repair cannot be made immediately, the hazard is guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired: (a) On the first floor there was an unguarded damaged section of the floor approximately ten (10) feet by twelve (12) feet where the supporting structure had rotted and collapsed the floor exposing employees to falls and struck-by hazards from debris. (b) Multiple locations throughout the facility: The wooden floor was splintered and damaged and not repaired exposing employees to slips trips, and falls.
