29 CFR 1910.29(b)(1): The employer did not ensure that guardrail systems met the requirement that the top edge height of top rails, or equivalent guardrail system members, that are 42 inches (107 cm), plus or minus 3 inches (8 cm), above the walking-working surface. The top edge height may exceed 45 inches (114 cm), provided the guardrail system meets all other criteria of paragraph (b) of this section (see Figure D-11 of this section). Mezzanine Stairs Landings: On or about March 15, 2021, two landings on the stairs leading up to the mezzanine had top rails that were measured and found to be approximately 35 inches in height, exposing employees to falls of approximately six feet, four inches and 12 feet respectively.

29 CFR 1910.304(f)(1)(iv):Overcurrent devices for circuits rated 600 volts, nominal, or less, were not readily accessible to each employee or authorized building management personnel: Fabrication Area near a large air compressor: On or about March 15, 2021, a disconnect switch was blocked by storage of a five gallon bucket as well as metal stock materials, prolonging employees exposure to energized equipment, in the event it would need to be de-energized.
