29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) On or about March 31, 2021 and times prior on the CO4 line, pull test section: Employees engaged in change-over activities involving 8-9 jigs were exposed to caught-in hazards in that the employer failed to ensure the machine specific energy control procedures were utilized to prevent the unexpected release of energy sources such as electric, pneumatic and hydraulic.

29 CFR  1910.147(c)(6)(i):The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  a) On or about March 30, 2021 and times prior the annual lockout/tagout periodic inspection: The employer failed to perform a periodic review of the energy control procedures to ensure employees conducting work on equipment, such as jig change over, were adequately securing hazardous energy on equipment in accordance with the written machine specific energy control procedure.

29 CFR  1910.212(a)(3)(ii):The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle:  a) On or about March 30, 2021 on the CO2 Clamper: The employer failed to ensure proper point-of-operation guarding was provided to protect the employee from having any part of their body in the danger zone during the operating cycle in that the light curtain was not connected to the clamping porting of the machine which allowed an employee to reach into the machine during the operation cycle.
