29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about March 31, 2021 and times prior on the CO4 line, pull test section: Employees engaged in change-over activities involving 8-9 jigs were exposed to caught-in hazards in that the employer failed to ensure the machine specific energy control procedures were utilized to prevent the unexpected release of energy sources such as electric, pneumatic and hydraulic.

The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle: a) On or about March 30, 2021 on the CO2 Clamper: The employer failed to ensure proper point-of-operation guarding was provided to protect the employee from having any part of their body in the danger zone during the operating cycle in that the light curtain was not connected to the clamping porting of the machine which allowed an employee to reach into the machine during the operation cycle.
