29 CFR  1910.147(c)(7)(i)(A):Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation.    On or about  March 8, 2021, in the planer area, an authorized employee was exposed to hazardous energy sources  while performing maintenance to machinery without receiving training in the recognition of applicable hazardous energy sources and methods and means necessary for energy isolation.

29 CFR  1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  On or about March 8, 2021, at a worksite located at 10001 FM 2025 in Cleveland, TX,  an employee was hospitalized as a result of a work-related injury that was not reported to OSHA within 24 hours.
