The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.   a) On or about March 26, 2021, the employer had not established a lockout tagout program with lockout procedures, training, and inspections for equipment such as but not limited to the General Corrugated Machinery Company Taper Gluer SN UTL350-3505 and the Hooper 50 Inch Press.

29 CFR  1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: (d)(1) Preparation for shutdown. Before an authorized or affected employee turns off a machine or equipment, the authorized employee shall have knowledge of the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy. (d)(2) Machine or equipment shutdown. The machine or equipment shall be turned off or shut down using the procedures established for the machine or equipment. An orderly shutdown must be utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage. (d)(3) Machine or equipment isolation. All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s).  a) On or about March 26, 2021, there were no lockout tagout procedures preparing for shutdown, shutting down the equipment, and isolating the energy sources for machines such as but not limited to the General Corrugated Machinery Company Taper Gluer SN UTL350-3505 and the Hooper 50 Inch Press for employees to follow performing setups and cleaning.

29 CFR  1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  a) On or about March 26, 2021 on the General Corrugated Machinery Company Taper Gluer SN UTL350-3505, employees were exposed inadequate guarding at the sides of the conveyor in that 1/3 of the conveyor was not guarded at the powered rotating wheels.  b) On or about March 26, 2021 on the General Corrugated Machinery Company Taper Gluer SN UTL350-3505, employees exposed to an ingoing nip point where the cardboard was fed by hand into the machine.  c) On or about March 26, 2021 on the Hooper 50 Inch Press, employees exposed to an ingoing nip point at the outfeed of the machine between the bottom roller and the shaft.
