29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee was using eye or face protection when exposed to eye or face hazards from flying particles.  At a jobsite located at 8920 North Oxford Avenue, Kansas City, Missouri 64157, employees were exposed to eye and face injuries from flying particle hazards. Employees conducting residential roofing work were observed working with assorted power tools without eye and face protection.

Employees conducting residential roofing work were observed working at elevation without utilizing fall protection systems.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At a jobsite located at 8920 North Oxford Avenue, Kansas City, Missouri 64157, employees were exposed to serious fall hazards. Employees conducting residential roofing work were observed accessing the roof by utilizing portable ladders. The portable ladder side rails did not extend at least 3 feet (.9 m) past the upper landing surface being accessed.
