A)     The employer provided fall protection equipment to employees, but among the equipment available, two lanyards were damaged, and had not been removed from service.

29 CFR  1910.215(a)(4):Work rests. On offhand grinding machines, work rests were not kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage.  A)     A bench grinder in the maintenance shop had its work rest 1/4 inch from the surface of the wheel.

29 CFR  1910.22(a)(1):(1) All places of employment, passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitary condition.  A)     Building 8 was a storage facility.  Part of the main storage area had material stored all along the floor making access to and from storage containers and racks difficult.

A)     In the salmon culture lab building.  There was a door along an exit access leading to the outside that was partially blocked by pipes.  It was not marked "Not an Exit".

29 CFR  1910.1200(f)(6)(ii): Workplace labeling. The employer did not ensure that each container of hazardous chemicals in the workplace is labeled, tagged or marked with product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals, and which, in conjunction with the other information immediately available to employees under the hazard communication program, will provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.  A)     In the maintenance shop, there was plastic container with a colored liquid in it, that was not labeled as to its contents.
