29 CFR  1926.502(d)(1): Connectors shall be drop forged, pressed or formed steel, or made of equivalent materials.  Everton at 3S520 Cambridge Ct., Warrenville, IL 60555, unmarked/unlabeled tie-off strap (single D-ring with a web loop end): on or about March 26, 2021, an employee engaged in residential roofing work was exposed to hazards associated with non-compatible use of a tie-off strap as an extension in his personal fall arrest system. The looped webbing (connector) of a tie-off strap was wrapped around a D-ring of his personal full-body harness, with the opposite end (single D-ring) connecting to a self-retracting lifeline.   There is no abatement certification or documentation required for this item.
