29 CFR  1926.800(g)(5)(ii):On jobsites where less than 25 employees worked underground at one time, the employer did not provide or arrange in advance for at least one 5-person rescue team:  On or about May 1, 2021, and at times prior to and thereafter, the employer did not ensure at least one 5-person rescue team was either onsite or within one-half hour travel time to the site. Employees were exposed to hazards including but not limited to air contaminants, struck-by, crushed-by, and water intrusion.
