29 CFR  1910.106(d)(5)(vi)(a): Where a storage building was located 50 feet or less from a building or line of adjoining property that may be built upon, the exposing wall was not a blank wall having a fire-resistance rating of at least 2 hours:  Instance a).  On or about April 7, 2021, at the K-Solv facility located in Channelview Texas, the employer did not ensure that storage warehouse building #2,  located less than 50 feet from the maintenance building #4, had an exposing wall with at least 2-hour fire resistance rating.   Instance b).  On or about April 7, 2021,  at the K-Solv facility located in Channelview Texas, the employer did not ensure that the storage warehouse building #3,  located less than 50 feet from the main office building #1, had an exposing wall with at least 2-hour fire resistance rating.

29 CFR  1910.106(e)(6)(ii): Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 degree F (37.8 degree C), were dispensed into containers without the containers being electrically interconnected:   On or about April 7, 2021, at the K-Solv facility located in Channelview Texas, the employer did not ensure that the transfer piping and 55-gallon metal drum were electrically interconnected (or bonded) while the flammable liquid mixture of naphthalene and toluene was dispensed into a 55-gallon drum from a 275-gallon tote in bay#1 area of the storage warehouse building #2.
