Employees were exposed to caught-in or crushed-by hazards.  a) The Sheeter Line 1 Masa Hopper was not guarded to prevent exposure. b) The Sheeter Line 2 Masa Hopper was not guarded to prevent exposure.

29 CFR  1910.305(g)(1)(iv)(C):Flexible cords and cables were run through doorways, windows, floors, or similar openings.  On or about April 9, 2021, and at times prior thereto, at the door entrance to the production department, the employer did not ensure that flexible electrical cords were not ran though doorways. Employees were exposed to electrical shock and or trip and fall hazards.

29 CFR  1910.39(c)(2):A fire prevention plan did not include procedures to control accumulations of flammable and combustible waste materials.  On or about April 9, 2021, and at times prior thereto, the employer did not ensure empty boxes filled with empty masa paper bags and other combustible waste materials were removed from the production area.  Employees were potentially exposed to burn hazards in the event of a fire.
