On or about April 13, 2021, at job site 418 E. Harrison, St., Elmhurst, IL 60126 , the employer failed to ensure that employees were trained to recognize and minimize fall hazards when working from scaffolds. Employees performing residential construction from a metal scaffold were exposed to fall hazards of approximately 12 feet when no fall protection system was utilized.
