29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about April  13, 2021, at job site 418 E. Harrison, St., Elmhurst, IL 60126, An employee  engaged in construction activities on floor level  was not provided  a hard hat.  Employees at levels of height were performing stone removal , while the  employee at ground level was exposed to head injuries from falling objects.

29 CFR  1926.451(b)(1):Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights.  On or about April  13, 2021, at job site 418 E. Harrison, St., Elmhurst, IL 60126, the  employer failed to ensure that scaffold planks were fully planked and installed, exposing employees to fall hazards of approximately 12 feet while standing on a metal scaffold engaged in construction activities.

29 CFR  1926.454(a)(1): The employer failed to have each employee who performs work while on a scaffold trained regarding the nature of any electrical hazards, fall hazards and falling object hazards in the work area.   On or about April  13, 2021, at job site 418 E. Harrison, St., Elmhurst, IL 60126 , the employer failed to ensure that employees were trained to recognize and minimize fall hazards when working from scaffolds.   Employees performing residential construction from a metal scaffold were exposed to fall hazards of approximately 12 feet when no fall protection system was utilized.
