This exposed employees to struck-by hazards.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles: On or about April 8, 2021, at the jobsite, the employer did not ensure that employees, using a powered saw, pneumatic nail gun, and hammers to install roof deck, were in use of eye protection. This exposed the employees struck-by eye injuries.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On or about April 8, 2021, at the jobsite, the employer did not ensure that the side rails of an extension ladder extended at least three(3) feet above the landing and was not tied off to prevent displacement. The employees were using the extension ladder to access the roof were exposed to a fall hazard.
