29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   (a) Wheat Flour Tortilla Production: On or about April 10, 2021, lockout tagout procedures were not utilized when operators were required to open and/or remove the catch pan of the Gemini Rounder Divider machines to clean dough from it while the machine was in operation; exposing employees to the crushing hazard created by the ram in motion.
