29 CFR  1910.67(c)(2)(v):  A personal fall arrest or travel restraint system that meets the requirements in 29 CFR part 1910, subpart I, shall be worn and attached to the boom or basket when working from an aerial lift:  On May 6, 2021 at a worksite located at 1751 Wildwood Drive, Suamico, WI 54173; an employee working from the basket of a truck mounted extensible boom platform aerial lift greater than 6 feet above the paved ground was not wearing a personal fall arrest system and was exposed to fall hazards.
