29 CFR 1910.28(b)(9)(i)(A): For fixed ladders that extend more than 24 feet (7.3 m) above a lower level, the employer did not ensure that each fixed ladder installed before November 19, 2018 is equipped with a personal fall arrest system, ladder safety system, cage, or well.  On or about May 13, 2021, employees were exposed to a fall hazard when utilizing a fixed ladder without tying off to the ladder using their fall arrest system.

29 CFR 1910.140(c)(18): The employer did not ensure that personal fall protection systems are inspected before initial use during each workshift for mildew, wear, damage, and other deterioration, and defective components are removed from service.  a. Employees were exposed to a fall hazard while utilizing Miller brand personal fall protection equipment without removing worn components from service.  b. Employees were exposed to a fall hazard while utilizing Show Wave brand personal fall protection equipment without removing worn components from service.
