29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system, or an alternative fall protection measure that meets a provision described in paragraph (b) of this section: a) Lot 9 of the Autumn Ridge Development in Milford, DE 19963: On or about May 13, 2021, the employer failed to ensure employees conducting roof loading work (setting bundles of shingles at the ridge of the roof) on a newly constructed property while being unprotected in all directions, approximately 20 feet above grade.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access: a) Lot 9 of the Autumn Ridge Development in Milford, DE 19963: On or about May 13, 2021, the employer failed to ensure employees used ladders that extended at least 3 feet above the upper landing to access the the upper levels of a multi story building, exposing employees to fall hazards approximately 10 feet above grade.
