29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system, or an alternative fall protection measure that meets a provision described in paragraph (b) of this section:  a) Lot 9 of the Autumn Ridge Development in Milford, DE 19963: On or about May 13, 2021, the employer failed to ensure employees conducting roof loading work (setting bundles of shingles at the ridge of the roof) on a newly constructed property while being unprotected in all directions, approximately 20 feet above grade.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways:  a) Lot 9 of the Autumn Ridge Development in Milford, DE 19963: On or about May 13, 2021, The employer did not provide training to employees working on or with various types of ladders to gain access to their work area approximately 23 feet above lower grade.
