29 CFR  1926.20(b)(2):The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s).  On or about May 13, 2021 and times prior thereto, the contractor site Superintendent did not ensure subcontracted workers employed in the performance of the contract work were free from recognized hazards, and did not correct all unsafe hazards, where workers were;  a)  working over six feet without fall protection and exposed to a fall hazard up to approximately 11 feet, and b) performing masonry work cutting material without wet tools, safety glasses and/or protective masks exposing them to silica hazards.
