29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  At the establishment, in the silk screen department, employees operating the Dubit #1 Silk Screen machine, model 306, serial number N22799 were exposed to amputation hazards.  Adequate machine guarding was not in place to prevent exposure to point of operation hazards.   Plastic Industries Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1305596, Citation Number 1, Item number 1, and was affirmed as a final order on August 29, 2018 with respect to a workplace located at  100 Lake Drive, Newark, DE 19702.
