The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tank cars, or motor vehicle cargo tanks shall be in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in Sec. 1910.6. On or about May 20, 2021 and times prior there to, the employer did not ensure that compressed gas cylinders were being stored securely and handled in accordance with the Compressed Gas Association. Employees exposed to injuries associated with explosions and struck by hazards: a) Pressurized acetylene tank stored near front of shop in the welding area

29 CFR 1910.147(c)(1):Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. On or about May 20, 2021 and times prior there to, the employer did not develop or implement written lockout/tagout procedures for each type of equipment or machine located within the manufacturing facility. Employees performing maintenance work on the equipment such as but not limited to the R-Panel, Slip and Cut and other press type machines were exposed to electrical shock and amputation hazards

29 CFR 1910.253(b)(2)(ii):Inside of buildings, cylinders shall be stored in a well-protected, well-ventilated, dry location, at least 20 (6.1 m) feet from highly combustible materials such as oil or excelsior. Assigned storage spaces shall be located where cylinders will not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons. On or about May 20, 2021 and times prior there to, employees were exposed to potential explosion hazards associated with a pressurized acetylene cylinder that was not chained or secured and was left free standing: a) Front of shop area where welding equipment and supplies were stored.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that the strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws. On or about May 20, 2021 and times prior there to, employees were exposed to electroshock hazards caused by the main power flexible cable for the R-Panel Machine not being provided strain relief. The power cable had exposed wiring due to the friction and movement of being unplugged without being provided with strain relief.
