29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).  a) Prana Imports, Inc. dba Prana Gourmet Foods/625 Pierce St. Ste A Somerset, NJ 08873 - Warehouse: - Forklift operators were not trained and evaluated to safely operate powered industrial trucks at the worksite. To include, but not limited to: -Truck safety related topics including the use of operator fall restraint systems and workplace related topics; -Evaluation of the operator's performance in the workplace.  Condition noted on or about 5/21/2021.

The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.  a) Prana Imports, Inc. dba Prana Gourmet Foods/625 Pierce St. Ste A Somerset, NJ 08873 - Warehouse: Certification was not done for all employees operating Forklift Trucks.
