29 CFR  1910.37(a)(3):Exit route(s) were not kept free and unobstructed:  a) Richmond University Medical Center, Staten Island, NY/ Adult Psychiatric Unit.-The exit door behind the nursing station was obstructed by a plastic garbage container and paper bags that consisted of patients personal belongings.  Observed On or about 5/18/2021.

29 CFR  1904.8(a):  Employee(s) names were not removed from the 300 Log when employee(s) experienced needle stick and sharps injuries:  a)) Richmond University Medical Center/ 355 Bard Ave  Staten Island, NY site: On OSHA 300 logs for 2021 and previous years, employees names were entered on the 300 log form for sustaining needle stick injuries instead of using the "Privacy Case" designation. Instances include but are not limited to entries in 2021 involving a finger stick incident while an employee was involved in vaccination activity.  Observed on or about  5/18/2021.

29 CFR  1904.29(b)(2):The employer did not fill out or correctly fill out an OSHA Form 301 or equivalent for each recordable injury or illness.   a) Richmond University Hospital, Staten, 355 Bard Ave, Staten Island,  NY- The employer did not fill out the OSHA form  for the years 2020 for the following work-related injuries or illnesses were not filled out. Some of the 301 's were missing or filled out incorrectly and did not specify the work related injuries.  Observed on or about on 10/14/2021.
