29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial trucks were performed by the employer without the manufacturer's prior written approval:   a) Warehouse - On or about May 25, 2021, the employer failed to ensure that the manufacturer's prior written approval was obtained before performing modifications which could affect capacity and safe operation of a powered industrial trucks, such as the forklift platform safety cage attachment used with the  Crown forklift, model #RM1025-45, serial #1A525530.

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:   a) Warehouse - On or about May 25, 2021, the employer did not mark the Crown forklift, model #RM1025-45, serial #1A525530, to identify the yellow safety cage working platform attachment and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered.
