29 CFR  1910.132(d)(2): The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed:  a) American EMS LLC located at 12755 Veterans Memorial Hwy, Douglasville, GA 30134:   On or about May 28, 2021, employees were exposed to respiratory and bloodborne pathogen hazards in that the employer did not certify that a hazard assessment had been performed to determine the appropriate personal protective equipment for protecting employees from hazards specific to their workplace.

29 CFR  1910.134(c)(2)(i):Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:  a) American EMS LLC located at 12755 Veterans Memorial Hwy, Douglasville, GA 30134:   On or about May 28, 2021, and at times prior, the employer exposed employees to respiratory hazards in that employees who wore N95 respirators voluntarily were not provided with the information contained in Appendix D regarding the respirators' appropriate use and maintenance.
