29 CFR  1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.   a) Fletchers Towing Service located at 20615 US 23 N, Circleville, Ohio 43113:  On and preceding May 14, 2021, the employer did not ensure that a work-related incident resulting in a hospitalization was reported within 24 hours. At approximately 8:00 pm on 5/14/2021, an on-duty tow truck operator was injured on the job and hospitalized.  The employer did not report the hospitalization to the OSHA office until 5/28/2021 at 1:00 pm.

29 CFR  1910.132(d)(2): The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed:  a) Fletchers Towing Service located at 20615 US 23 N, Circleville, Ohio 43113:  On and preceding May 14, 2021, the employer did not verify, through a written certification, that the required workplace hazard assessment had been performed.  The employer did not certify, in writing, a workplace hazard assessment for tow truck operators performing tasks which necessitate the use of personal protective equipment including but not limited to the use of gloves and retro reflective outerwear.
