29 CFR  1910.28(b)(8)(ii): 29 CFR 1910.28(b)(8)(ii): For a repair pit, service pit, or assembly pit that was less than 10 feet (3 m) deep, the employer did not apply floor markings at least 6 feet (1.8 m) from the edge of the pit in colors that contrast with the surrounding area; or place a warning line at least 6 feet (1.8 m) from the edge of the pit as well as stanchions that are capable of resisting, without tipping over, a force of at least 16 pounds (71 N) applied horizontally against the stanchion at a height of 30 inches (76 cm); or place a combination of floor markings and warning lines at least 6 feet (1.8 m) from the edge of the pit.     a) Travel Centers of America, LLC located at 940 SR 42 North, London, OH  43140:  On and preceding June 8, 2021, the employer did not ensure that floor markings or a combination of floor markings and warning lines were in place where employees worked near open repair pits that were approximately 50 inches deep.   Bay 6 was a full length repair pit where several technicians performed various maintenance tasks on semi trucks.  The bay 6 pit had grates that were periodically removed and no floor markings or warning lines in place which exposed employees to falls into the pit.  b) Travel Centers of America, LLC located at 940 SR 42 North, London, OH  43140:  On and preceding June 8, 2021, the employer did not ensure that floor markings or a combination of floor markings and warning lines were in place where employees worked near open repair pits that were approximately 50 inches deep.   Bay 5 was a half-length repair pit where several technicians performed various maintenance tasks on semi trucks.  The bay 5 pit had grates that were periodically removed and no floor markings or warning lines in place which exposed employees to falls into the pit.

29 CFR  1910.28(b)(8)(iii): 29 CFR 1910.28(b)(8)(iii): For a repair pit, service pit, or assembly pit that was less than 10 feet (3 m) deep, the employer did not post readily visible caution signs that meet the requirements of �1910.145 and state "Caution-Open Pit."   a) Travel Centers of America, LLC located at 940 SR 42 North, London, OH  43140:  On and preceding June 8, 2021, the employer did not post readily visible caution signs which stated "Caution-Open Pit" where employees worked near open repair pits that were approximately 50 inches deep.   Bay 6 was a full length repair pit where several technicians performed various maintenance tasks on semi trucks.
