On or about  June 5, 2021, the employer failed to ensure energy control procedures was developed and documented for the control of hazardous energy when an employee was engaged in a service and/or maintenance task, such as adjusting the check-weigher chain tension on assembly line 6.

29 CFR  1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:  a) On or about June 5, 2021, on line 6, the employer failed to provide training to each authorized employee performing service and/or maintenance to machinery or equipment to ensure employee acquired the knowledge and skills required for the safe application, usage, and removal of energy control devices.  Maintenance workers have not received training on specific LOTO procedures for repairing  and conducting maintenance on line 6.
